A dermoid cyst is a congenital, benign growth that develops when skin and its associated structures become trapped beneath the surface during embryonic development. Unlike simple fluid-filled cysts, dermoid cysts contain fully differentiated tissue elements including hair follicles, sebaceous glands, sweat glands, and keratinized epithelium. These cysts are classified as choristomas, meaning they consist of normal tissue located in an abnormal anatomical position.
Dermoid cysts form during the embryonic period when the ectoderm, the outermost germ layer that gives rise to skin and neural tissue, fails to separate properly. Small nests of ectodermal cells become sequestered beneath the developing skin surface, where they continue to differentiate and grow into organized skin-like structures. The result is an encapsulated mass that essentially functions as a pocket of displaced skin buried within deeper tissue layers.
These cysts are typically present at birth, though they may not become clinically apparent until weeks, months, or even years later as they slowly enlarge. The growth rate of dermoid cysts is generally slow and corresponds to the natural turnover of the skin elements they contain. As hair grows and sebaceous glands produce oil within the enclosed space, the cyst gradually expands with accumulated keratinous debris and oily secretions.
Dermoid cysts are considered developmental anomalies rather than true neoplasms. They do not undergo malignant transformation and do not metastasize. However, their presence can cause discomfort, secondary infection, or complications depending on their anatomical location, making veterinary evaluation and treatment advisable in most cases.
