Cholesterosis bulbi is a degenerative ocular condition in dogs characterized by the accumulation of cholesterol crystals within the structures of the eye. These crystalline deposits most commonly form in the vitreous humor, the gel-like substance that fills the posterior chamber of the eye, though they may also appear in the anterior chamber or within the subretinal space. The condition typically develops as a secondary complication following chronic intraocular inflammation, trauma, or hemorrhage, rather than occurring as a primary disease process.
The term synchysis scintillans is frequently used interchangeably with cholesterosis bulbi, though it more precisely describes the specific phenomenon of freely mobile cholesterol crystals suspended within a liquefied vitreous. When the vitreous degenerates and loses its normal gel-like consistency through a process called synchysis, cholesterol crystals that have precipitated within the eye become free to move and settle with gravity. This creates a distinctive clinical appearance that veterinary ophthalmologists can readily identify during examination.
Cholesterosis bulbi is not considered a common condition in veterinary practice, but it is well recognized among veterinary ophthalmologists as a sequela of various ocular diseases. The condition itself may not always cause significant visual impairment, particularly in its early stages. However, because it almost invariably indicates underlying or prior ocular pathology, its presence warrants thorough ophthalmic evaluation to identify and address the root cause of the crystal deposition.
The pathophysiology of cholesterosis bulbi involves the breakdown of cell membranes and blood components within the eye. When hemorrhage or inflammation occurs within the ocular chambers, red blood cells and other cellular components degrade over time. Cholesterol, a major constituent of cell membranes, is released during this degradation process. When the local concentration of cholesterol exceeds the capacity of the ocular fluids to maintain it in solution, it precipitates out as crystalline deposits that accumulate progressively.
