Carpal hyperextension in dogs is a debilitating orthopedic condition characterized by breakdown of the normal supporting structures of the carpus (wrist), resulting in collapse of the joint into an abnormally extended position. Under normal circumstances, the carpus maintains a slightly flexed angle that allows dogs to walk, run, and bear weight on their forelimbs. This normal angle is maintained by a complex system of ligaments, particularly the palmar carpal ligaments and palmar fibrocartilage, which prevent the joint from collapsing backward. When these critical support structures are damaged or fail, the carpus hyperextends, causing the dog to walk with the wrist flat to the ground or nearly so, a condition sometimes described as dropped carpus or plantigrade stance of the forelimb.
The carpus is one of the most complex joints in the canine body, composed of seven carpal bones arranged in two rows between the radius and ulna above and the metacarpal bones below. Three separate joint levels exist within this small anatomical area: the antebrachiocarpal joint between the forearm and carpus, the middle carpal joint between the two rows of carpal bones, and the carpometacarpal joints between the carpus and the metacarpals. Ligaments on the palmar (back) surface of the joint bear enormous loads during normal weight-bearing and must resist the forces that would otherwise hyperextend the joint. Damage to these structures at any level can cause joint instability and collapse.
Carpal hyperextension most commonly results from traumatic injury, particularly falls from significant heights or jumping down from elevated surfaces. The impact forces generated during landing can exceed the strength of the palmar ligamentous structures, causing acute rupture. In other cases, degenerative weakening of the ligaments allows gradual progressive hyperextension without a single identifiable traumatic event. Bilateral carpal hyperextension affecting both front legs may suggest systemic factors such as immune-mediated polyarthritis or underlying collagen abnormalities. Understanding the cause is important for treatment planning and prognosis.
The prognosis for carpal hyperextension depends on prompt diagnosis and appropriate surgical treatment. Conservative management with splinting or casting is uniformly unsuccessful in providing long-term functional improvement, as the damaged ligaments cannot heal adequately to restore stability. Surgical treatment, typically involving fusion (arthrodesis) of the affected joint levels, is necessary to restore the ability to walk comfortably. While surgery eliminates normal joint motion, dogs compensate remarkably well and return to comfortable, functional lives. Early surgical intervention before secondary joint damage occurs generally yields the best outcomes.
