An arteriovenous fistula is an abnormal connection between an artery and a vein that bypasses the normal capillary network, allowing blood to flow directly from the high-pressure arterial system into the low-pressure venous system. In healthy dogs, blood flows from arteries through progressively smaller vessels until reaching tiny capillaries where oxygen and nutrients exchange with tissues before draining into veins. When an arteriovenous fistula exists, this orderly flow is disrupted, causing blood to take a shortcut that can have significant hemodynamic consequences. These vascular abnormalities can occur anywhere in the body but are most commonly identified in the limbs, liver, and head and neck region in dogs.
The pathophysiology of arteriovenous fistulas involves the shunting of blood from the arterial system directly into veins without passing through the resistance-providing capillary bed. This abnormal pathway creates several problems including increased blood flow through the affected area, elevated pressure in the venous system receiving the shunted blood, and reduced blood flow to tissues downstream from the fistula. The heart must work harder to maintain adequate circulation, as blood continuously cycles through the low-resistance fistula rather than perfusing tissues effectively. Over time, the increased cardiac workload can lead to heart enlargement and potentially heart failure if the fistula is large or has been present for an extended period.
Arteriovenous fistulas in dogs may be congenital, present from birth due to developmental abnormalities in blood vessel formation, or acquired through trauma, surgery, or other injury to blood vessels. Congenital forms often involve the liver, where hepatic arteriovenous fistulas can cause significant clinical problems, or may occur in the limbs or other locations. Acquired fistulas typically result from penetrating injuries, surgical complications, or other trauma that creates an abnormal connection between adjacent arteries and veins. The clinical presentation varies widely depending on the location, size, and duration of the fistula, ranging from incidental findings to life-threatening complications.
Diagnosis and treatment of arteriovenous fistulas require specialized imaging and often surgical or interventional expertise. Clinical signs may prompt investigation, or fistulas may be discovered during evaluation for other problems. Advanced imaging techniques including Doppler ultrasound, computed tomography angiography, and conventional angiography help characterize the abnormal vascular connections and plan treatment. Treatment options include surgical ligation or excision, interventional embolization procedures, or in some cases careful monitoring when risks of treatment outweigh benefits. Veterinary specialists including surgeons and cardiologists play important roles in managing these complex vascular abnormalities, working to restore normal blood flow patterns and prevent long-term cardiac consequences.
