Urinary sludge, accumulation of calcium-rich sediment in the bladder, often coexists with stones and contributes to stone formation. Urinary tract infections can both predispose to stone formation by providing a nidus for crystallization and develop secondary to obstruction and catheterization. Chronic kidney disease may result from obstruction-related damage or occur as a concurrent condition in older guinea pigs.

Conditions with similar symptoms must be differentiated from urethral obstruction. Bladder stones without obstruction cause straining and urinary symptoms but the guinea pig can still urinate. Urinary tract infections cause frequent urination attempts, vocalization, and discomfort. Bladder or urethral tumors can cause urinary symptoms and potential obstruction. Constipation can cause straining that may be confused with urinary straining. Reproductive tract conditions in both males and females can affect urination. Musculoskeletal pain may alter posture and be misinterpreted as urinary difficulty. The key distinguishing feature of complete obstruction is the distended bladder and inability to produce urine.

Potential complications of urethral obstruction and its treatment are significant. Acute kidney injury is nearly universal with complete obstruction and may progress to chronic kidney disease in severe cases. Bladder atony, loss of normal bladder tone and function, can persist after severe overdistension. Urethral stricture may develop after catheterization or surgery, predisposing to future obstruction. Post-obstructive diuresis can cause dangerous fluid and electrolyte shifts during recovery. Infection can complicate catheterization and surgery. Recurrence of stones and obstruction is common without strict management. GI stasis can develop secondary to anorexia and pain, creating a concurrent emergency. Understanding these potential complications helps owners and veterinarians monitor appropriately and intervene early.
