Tooth root abscesses, molar malocclusion, and elongated tooth roots create pathways for infection to reach the orbit. Guinea pigs with panophthalmitis should always undergo thorough dental evaluation even after the acute ocular crisis is addressed. Other concurrent conditions may include respiratory infections, as some of the same bacterial organisms can affect both systems. Vitamin C deficiency may be present, contributing to impaired immune function and wound healing. Gastrointestinal stasis frequently develops secondary to anorexia from pain and illness, requiring concurrent treatment.

Conditions with similar initial presentation that must be differentiated from panophthalmitis include retrobulbar abscess, which causes eye protrusion and facial swelling but may not involve the interior of the eye to the same degree initially. Orbital cellulitis affects tissues around the eye without necessarily involving the globe. Severe conjunctivitis or keratitis can cause significant discharge and discomfort but typically lacks the systemic signs of panophthalmitis. Acute glaucoma causes painful eye enlargement but is not primarily infectious. Hyphema (blood in the anterior chamber) following trauma may appear similar but has different characteristics on examination. Accurate differentiation is essential as treatment approaches and prognoses differ significantly.

Potential complications of panophthalmitis beyond the affected eye itself include septicemia, or bloodstream infection, which can occur when bacteria spread from the localized eye infection throughout the body. This represents a life-threatening emergency requiring intensive treatment. Osteomyelitis, or bone infection, may develop in the orbital bones or skull. Extension of infection to the brain can cause meningitis or brain abscess, manifesting as neurological signs including head tilt, circling, seizures, or altered consciousness. Chronic sinus infection may develop if nasal passages are involved. Even with successful treatment of the eye, underlying dental disease may progress and cause future problems if not adequately addressed.
