Zygomatic salivary gland disease causes facial swelling that may be confused with orbital pathology. Trauma causing orbital hemorrhage or swelling can produce acute eye protrusion. Careful clinical examination, imaging studies, and sometimes tissue sampling help distinguish between these possibilities, though overlapping conditions are possible.

Potential complications of retrobulbar abscess are serious and numerous. Local complications include permanent damage to the eye from compression or infection, potentially requiring enucleation even after the abscess resolves. Damage to orbital muscles may cause persistent strabismus or abnormal eye position. Extension of infection to the brain can cause meningitis or brain abscess, manifesting as neurological signs and carrying high mortality. Septicemia from bloodstream spread of bacteria causes systemic inflammatory response and potential multi-organ failure. Bone infection (osteomyelitis) of orbital or facial bones may develop and is difficult to eradicate. Gastrointestinal stasis secondary to anorexia can itself become life-threatening. Recurrence after apparently successful treatment is common if underlying dental disease persists.
