Hydronephrosis is a condition characterized by swelling and dilation of the kidney due to the accumulation of urine that cannot drain properly into the bladder. In guinea pigs, this condition occurs when urine flow is obstructed somewhere between the kidney and the bladder, causing urine to back up into the kidney and stretch the renal pelvis and collecting system. The increased pressure from accumulated urine damages the delicate kidney tissue, and if left untreated, can lead to permanent loss of kidney function in the affected organ. Hydronephrosis may affect one kidney (unilateral) or both kidneys (bilateral), with bilateral involvement being particularly serious.
Hydronephrosis is not among the most commonly diagnosed conditions in guinea pigs, but it occurs with notable frequency in the context of urinary tract disorders, particularly those involving stones or tumors. Guinea pigs are uniquely susceptible to urinary calculi due to their calcium metabolism, and stones lodging in the ureters are a common cause of hydronephrosis. The condition may also result from tumors compressing or invading the ureters, congenital abnormalities of the urinary tract, or scarring from previous infections or surgeries. Any guinea pig with known urinary tract problems is at potential risk for developing hydronephrosis.
The impact of hydronephrosis on a guinea pig's health depends on whether one or both kidneys are affected and how quickly the obstruction is addressed. Unilateral hydronephrosis may go undetected for some time if the other kidney compensates, though the affected kidney progressively loses function. Bilateral hydronephrosis or hydronephrosis in a guinea pig with pre-existing kidney disease is much more serious, as it can lead to acute kidney failure. Affected guinea pigs may experience pain, changes in urination, lethargy, appetite loss, and signs of kidney failure if the condition progresses. The longer the obstruction persists, the less likely the affected kidney is to recover function.
Early detection and treatment of hydronephrosis offer the best chance for preserving kidney function and maintaining the guinea pig's health. When obstruction is identified and relieved quickly, before significant kidney damage has occurred, the affected kidney may recover substantial function. Delayed treatment results in progressive, often irreversible, destruction of kidney tissue. Regular veterinary care and prompt attention to any signs of urinary problems are essential for early identification of hydronephrosis. Guinea pig owners should understand that this is a serious condition requiring specialized veterinary care, but with appropriate treatment, many affected guinea pigs can maintain good quality of life.
