Stroke, also known as cerebrovascular accident, occurs when blood flow to a portion of the brain is interrupted or when bleeding occurs within the brain. In cats, strokes are classified as either ischemic, resulting from blockage of blood vessels, or hemorrhagic, caused by rupture of blood vessels and subsequent bleeding into brain tissue. While once thought to be rare in cats, improved diagnostic imaging has revealed that feline strokes occur more commonly than previously recognized. Strokes can affect cats of any age but are most common in middle-aged to older cats, particularly those with underlying health conditions that affect blood vessels or blood clotting.
The causes of stroke in cats differ from those commonly seen in humans. Unlike human strokes, which often result from atherosclerosis and traditional cardiovascular risk factors, feline strokes typically occur secondary to other diseases. Heart disease, particularly hypertrophic cardiomyopathy, can lead to blood clot formation that travels to the brain. Hyperthyroidism, chronic kidney disease, and high blood pressure are common underlying conditions associated with feline strokes. Cancer, certain infections, and blood disorders can also predispose cats to stroke. In some cases, no underlying cause is identified despite thorough investigation, a situation termed cryptogenic stroke.
The impact of stroke on affected cats can range from mild to devastating depending on the location and extent of brain damage. Cats may experience sudden onset of neurological symptoms including loss of balance, circling, head tilt, abnormal eye movements, and altered mental status. Some cats become unable to walk or stand, while others may experience seizures or loss of consciousness. The sudden nature of symptom onset is often alarming to cat owners, who may initially believe their cat is dying. While strokes can be life-threatening, many cats survive and experience significant recovery with appropriate care and time.
Treatment of feline stroke focuses on supportive care during the acute phase and identification and management of any underlying conditions that contributed to the stroke. Unlike human medicine, where clot-dissolving drugs may be administered for ischemic strokes, such acute interventions are rarely performed in cats due to timing constraints and different underlying causes. The good news is that many cats show remarkable recovery from stroke, with the brain demonstrating significant capacity to compensate for damaged areas. Prognosis depends on the severity of the stroke, the underlying cause, and how well any concurrent diseases can be managed. Early veterinary intervention and appropriate ongoing care give cats the best chance at recovery.
