Retained deciduous teeth, also known as persistent primary teeth, occur when a cat's baby teeth fail to fall out as the permanent adult teeth erupt. Cats, like humans and many other mammals, are diphyodont, meaning they develop two sets of teeth during their lifetime. The deciduous (baby) teeth normally begin erupting at approximately two to three weeks of age and are fully present by six to eight weeks. These primary teeth should begin falling out around three to four months of age as the permanent teeth emerge, with the transition complete by approximately six to seven months of age. When deciduous teeth remain in place alongside erupting permanent teeth, problems can develop.
Retained deciduous teeth occur when the normal process of primary tooth root resorption fails to occur properly. Normally, as the permanent tooth develops and begins to erupt, pressure from the erupting tooth triggers resorption of the deciduous tooth's root, causing it to loosen and fall out. When this process fails, the deciduous tooth remains firmly attached, and the permanent tooth must erupt alongside it rather than in its place. This results in the characteristic appearance of two teeth occupying the space meant for one, often described as a double row of teeth. The condition is more common in small breed dogs but does occur in cats, particularly certain purebred lines.
The impact of retained deciduous teeth ranges from minor crowding to significant dental and jaw problems. When two teeth occupy the space designed for one, overcrowding creates areas that trap food and debris, promoting plaque accumulation and predisposing to periodontal disease. The abnormal positioning can cause malocclusion, where the teeth do not align properly when the mouth closes, potentially causing trauma to oral soft tissues. The permanent tooth may erupt at an abnormal angle or position, further complicating dental alignment. In some cases, the abnormal forces from crowded teeth can cause jaw development problems or damage to adjacent teeth.
The good news is that retained deciduous teeth are readily treatable through surgical extraction. When identified early, extraction of the retained primary teeth allows the permanent teeth to shift into proper position and prevents secondary complications. The procedure is typically straightforward when performed before significant complications develop. Veterinary examination during the teething period, around three to six months of age, allows identification and timely treatment of retained deciduous teeth. Cats recover quickly from these extractions and go on to have normal, healthy adult dentition when the condition is addressed appropriately.
