Pyloric stenosis is a condition in cats characterized by abnormal narrowing of the pylorus, the muscular valve at the lower end of the stomach that controls the passage of food into the small intestine. In healthy cats, the pyloric sphincter opens and closes rhythmically to allow properly digested stomach contents to move into the duodenum at an appropriate rate. When pyloric stenosis occurs, this opening becomes abnormally narrowed or fails to relax properly, impeding the normal flow of food out of the stomach. This obstruction causes food to accumulate in the stomach, leading to characteristic symptoms of chronic vomiting that typically occurs several hours after eating. The condition can be congenital, present from birth, or acquired later in life due to chronic inflammation, scarring, or tumor growth.
The causes of pyloric stenosis differ between congenital and acquired forms. Congenital pyloric stenosis results from abnormal development of the pyloric region during fetal growth, causing excessive thickening of the muscle or mucosal layers. This form typically becomes apparent in young cats, often within the first few months of life as they transition to solid food. Acquired pyloric stenosis develops in adult cats and can result from chronic gastritis causing thickening of the pyloric tissues, formation of polyps or masses in the pyloric region, or scarring from previous ulceration or injury. Certain breeds, particularly Siamese and related Oriental breeds, appear to have a genetic predisposition to developing pyloric abnormalities.
The impact of pyloric stenosis on affected cats ranges from mild inconvenience to severe, life-threatening complications depending on the degree of obstruction. Cats with partial obstruction may experience intermittent vomiting and mild weight loss while maintaining reasonable overall health. Complete or near-complete obstruction causes severe, persistent vomiting with an inability to retain any food, leading to rapid weight loss, dehydration, and electrolyte imbalances. The chronic nature of the condition causes progressive nutritional decline if not addressed. The accumulated stomach contents and persistent gastric distension cause discomfort and can lead to secondary complications including aspiration pneumonia, gastric ulceration, and metabolic disturbances.
Pyloric stenosis is generally treatable, with surgical correction being the most definitive approach for significant obstructions. Medical management may help cats with mild disease or stabilize patients before surgery, but surgery remains the treatment of choice for most clinically affected cats. The prognosis following appropriate surgical correction is generally good, with many cats returning to normal eating and quality of life. Early diagnosis before severe malnutrition or complications develop improves outcomes significantly. Any cat experiencing chronic vomiting, particularly vomiting of undigested or partially digested food several hours after eating, should receive veterinary evaluation to identify the cause and begin appropriate treatment.
