Chronic kidney disease may predispose cats to pyelonephritis while also resulting from repeated or severe kidney infections. Diabetes mellitus significantly increases infection risk through multiple mechanisms and commonly coexists with pyelonephritis. Management of cats with these co-occurring conditions requires addressing all aspects of their health comprehensively rather than treating each condition in isolation.

Several conditions present with symptoms similar to pyelonephritis and must be differentiated through diagnostic testing. Acute kidney injury from toxin exposure, dehydration, or other causes produces similar signs of kidney dysfunction without bacterial involvement. Chronic kidney disease in its various stages can cause many of the same symptoms. Ureteral obstruction from stones or strictures creates acute kidney pain and dysfunction. Lower urinary tract infections without kidney involvement share some symptoms but typically lack the systemic illness associated with pyelonephritis. Kidney cancer or other masses can cause similar imaging findings and clinical signs. Accurate diagnosis is essential because treatment approaches differ significantly among these conditions.

Potential complications of pyelonephritis include both immediate and long-term consequences that owners and veterinarians must monitor for. Sepsis, where bacteria spread from the kidney infection into the bloodstream, represents the most serious acute complication and can be life-threatening without immediate intensive treatment. Kidney abscess formation may occur when pockets of infection become walled off within kidney tissue, potentially requiring surgical intervention. Chronic or recurrent pyelonephritis can lead to progressive kidney scarring and eventual chronic kidney disease. Hypertension may develop secondary to kidney damage. Preventing complications through prompt, appropriate treatment and careful monitoring during recovery reduces the likelihood of these serious outcomes and preserves long-term kidney function and overall health.
