Hydronephrosis is a condition characterized by the abnormal dilation and swelling of the kidney's renal pelvis and calyces due to the accumulation of urine that cannot drain properly into the bladder. This backup of urine occurs when there is an obstruction or blockage somewhere in the urinary tract, most commonly in the ureter, which is the thin tube connecting the kidney to the bladder. In cats, hydronephrosis can affect one kidney (unilateral) or both kidneys (bilateral), with the severity ranging from mild dilation that may be incidentally discovered to severe distension causing significant kidney damage. The condition is not uncommon in feline practice and represents an important cause of kidney dysfunction that, unlike some other kidney diseases, may be surgically correctable if addressed promptly.
The underlying cause of hydronephrosis is always some form of obstruction preventing normal urine flow from the kidney to the bladder. In cats, the most common causes include ureteral stones (ureterolithiasis), which have become increasingly recognized as a significant feline health problem. Other causes include strictures or scarring of the ureter, blood clots, tumors compressing or invading the ureter, and congenital abnormalities of the urinary tract. Less commonly, external compression from masses or adhesions following abdominal surgery can lead to ureteral obstruction. The obstruction causes urine to back up into the kidney, progressively stretching the renal pelvis and eventually damaging the functional kidney tissue through increased pressure.
The impact of hydronephrosis on a cat's health depends significantly on whether one or both kidneys are affected and how quickly the obstruction developed. When only one kidney is affected and the obstruction develops gradually, cats may show minimal symptoms because the other kidney compensates for the lost function. However, bilateral hydronephrosis or obstruction affecting a cat's only functional kidney rapidly leads to kidney failure with potentially life-threatening consequences. Even in unilateral cases, the affected kidney can lose function permanently if the obstruction is not relieved in time, with some studies suggesting that complete obstruction can cause irreversible damage within one to two weeks. Chronic partial obstruction may cause progressive kidney damage over months.
Hydronephrosis is often treatable, particularly when detected early before permanent kidney damage has occurred. Treatment typically involves relieving the obstruction through surgical intervention, which may range from minimally invasive procedures to traditional surgery depending on the cause and location of the blockage. The success of treatment depends on the cause of obstruction, duration of the condition, degree of kidney damage at the time of diagnosis, and whether one or both kidneys are affected. Early detection through imaging studies when cats present with urinary symptoms or unexplained kidney changes significantly improves outcomes. Cat owners should understand that any urinary symptoms in cats warrant prompt veterinary evaluation, as urinary tract problems can rapidly become serious in felines.
