Cats with one type of allergy are at increased risk for developing additional allergies over time, and many chronically affected cats eventually prove sensitive to multiple allergens requiring multimodal management approaches.

Several conditions can mimic the appearance of miliary dermatitis and must be differentiated through diagnostic testing. Dermatophytosis, or ringworm, can produce crusty lesions that resemble miliary dermatitis, particularly in young cats or immunocompromised individuals. Cheyletiella mites cause a papular eruption with intense itching that may be confused with allergic miliary dermatitis. Bacterial pyoderma can produce pustular and crusty lesions as either a primary condition or secondary to allergic disease. Pemphigus foliaceus, an autoimmune skin disease, occasionally presents with a distribution pattern similar to miliary dermatitis. Drug reactions can sometimes manifest as miliary eruptions. Accurate differentiation among these conditions is essential for appropriate treatment selection.

Complications can develop when miliary dermatitis is left untreated or poorly controlled. Secondary bacterial infections are common, as damaged skin barriers allow bacteria to colonize and proliferate. These infections can progress to deep pyoderma or abscess formation if not addressed. Chronic self-trauma from scratching can lead to permanent scarring or areas of thickened, darkened skin. Severe hair loss may occur in heavily affected areas, and in some cases, hair regrowth may be incomplete. Chronic stress from persistent itching can affect the cat's behavior and overall wellbeing. Rarely, severe allergic reactions may trigger systemic symptoms beyond skin involvement. Early diagnosis and consistent management help prevent these complications and maintain both skin health and overall quality of life.
