Feline chronic gingivostomatitis, commonly abbreviated as FCGS and also known simply as stomatitis, is a severe, painful, and often debilitating oral inflammatory condition in cats. This disease is characterized by marked inflammation of the gingiva (gums) and oral mucosa that extends beyond the teeth to involve the caudal oral cavity, including the tissues at the back of the mouth near the throat. Unlike simple gingivitis, which is limited to inflammation at the gum line, FCGS involves severe, widespread inflammation that causes intense oral pain and significantly impacts affected cats' ability to eat, groom, and maintain normal quality of life. The condition affects an estimated one to twelve percent of cats presented to veterinary hospitals for dental problems, making it a significant feline health concern.
The exact cause of feline chronic gingivostomatitis remains incompletely understood, but the condition is believed to result from an inappropriate, exaggerated immune response to bacterial plaque antigens on tooth surfaces. The cat's immune system overreacts to the presence of oral bacteria, triggering severe inflammation that fails to resolve through normal means. Contributing factors may include viral infections, particularly feline calicivirus, feline leukemia virus, and feline immunodeficiency virus. Genetic factors appear to play a role, as certain individuals and possibly certain breeds show increased susceptibility. The condition is not caused by poor dental hygiene alone, though oral bacteria are central to triggering and perpetuating the inflammatory response.
The impact of feline chronic gingivostomatitis on affected cats is profound and often devastating. The severe oral pain prevents normal eating, leading to weight loss, malnutrition, and decline in overall health. Cats may approach food hungrily but then refuse to eat or eat only small amounts before walking away. Grooming becomes painful, leading to an unkempt coat. Many cats become withdrawn, hide, and resist interaction. The constant pain affects behavior, mood, and quality of life in ways that extend far beyond the oral cavity. Without effective treatment, affected cats suffer greatly, and some owners eventually choose euthanasia due to the severity of suffering and frustration with failed treatments.
Treatment of feline chronic gingivostomatitis presents significant challenges, but hope exists for most affected cats. The most effective treatment is full mouth or near-full mouth tooth extraction, which removes the tooth surfaces that trigger the immune response. Approximately sixty to eighty percent of cats show significant improvement following extraction, with many achieving cure or long-term remission. Cats that do not respond adequately to extraction may require additional immunomodulatory therapy. Medical management alone, without extraction, is rarely successful long-term but may be used when surgery is not possible. Early, aggressive treatment provides the best outcomes. Despite the challenges, many cats with FCGS go on to live comfortable, pain-free lives with appropriate treatment.
