Ehrlichiosis is a tick-borne infectious disease caused by bacteria of the genus Ehrlichia that primarily infect white blood cells, leading to immune system dysfunction and various clinical manifestations. While ehrlichiosis is well-documented and relatively common in dogs, it occurs only rarely in cats, making it an uncommon but important differential diagnosis for veterinarians evaluating feline patients with fever and blood abnormalities in tick-endemic areas. The organisms responsible for feline ehrlichiosis include Ehrlichia canis, Ehrlichia chaffeensis, and potentially other Ehrlichia species, though the exact species affecting cats may not always be identified. The disease has been reported sporadically in cats from various geographic regions where tick vectors are present.
The transmission of ehrlichiosis to cats occurs through the bite of infected ticks, primarily the brown dog tick (Rhipicephalus sanguineus) and the lone star tick (Amblyomma americanum), depending on the Ehrlichia species involved. When an infected tick feeds on a cat, the bacteria are transmitted through tick saliva and enter the bloodstream, where they invade white blood cells, particularly monocytes and granulocytes. Within these cells, the organisms multiply to form characteristic clusters called morulae, which can sometimes be visualized on blood smears. The incubation period from tick bite to clinical signs is typically one to three weeks, though cats may remain subclinically infected for extended periods.
The impact of ehrlichiosis on feline health varies considerably, ranging from subclinical infection with no apparent illness to significant systemic disease. Affected cats may develop fever, lethargy, and decreased appetite as initial signs. Blood abnormalities including low platelet counts, anemia, and white blood cell changes are common and may lead to bleeding tendencies or susceptibility to secondary infections. Some cats develop enlarged lymph nodes or spleen as the immune system responds to infection. While severe disease can occur, particularly in immunocompromised cats, the overall clinical impact in cats tends to be less severe than the disease commonly seen in dogs.
Treatment of feline ehrlichiosis is typically successful when appropriate antibiotic therapy is initiated promptly. Doxycycline is the antibiotic of choice and is usually effective at eliminating the infection within two to four weeks of treatment. Early recognition and treatment lead to the best outcomes, with most cats recovering fully without lasting effects. However, the rarity of the condition in cats means it may not be initially considered, potentially delaying diagnosis. Veterinary care is essential for proper diagnosis through blood testing and for ensuring appropriate treatment duration to fully clear the infection and prevent chronic carrier states.
