Some cats that experience drug eruption may later be diagnosed with or develop other autoimmune or immune-mediated conditions. Conversely, cats with existing autoimmune diseases may be receiving immunosuppressive medications that affect their response to other drugs. Comprehensive assessment of a cat's overall immune health may be warranted in cases of severe or unusual drug reactions.

Several conditions can be confused with drug eruption and must be distinguished for appropriate management. Primary autoimmune skin diseases, such as pemphigus, can produce lesions similar to those seen in drug eruption and may even be triggered by drugs in some cases. Infectious skin diseases, including bacterial pyoderma and dermatophytosis, can resemble some drug eruption patterns. Other allergic skin conditions, including atopic dermatitis and contact dermatitis, may produce similar lesions but have different causes and treatments. Neoplastic conditions occasionally cause skin changes that could initially be mistaken for drug reactions. Careful history taking, attention to the temporal relationship with medication use, and appropriate diagnostic testing help distinguish these conditions.

Complications of drug eruption can affect multiple organ systems in severe cases. Secondary bacterial infection of damaged skin is a common complication that requires antimicrobial treatment. In severe cases with extensive skin loss, fluid and electrolyte imbalances, hypothermia, and sepsis can develop. Drug hypersensitivity syndrome involves internal organ damage, most commonly affecting the liver, and can cause persistent illness even after drug discontinuation. Some drug reactions can trigger ongoing autoimmune processes that persist after the drug is stopped. Permanent scarring, pigmentation changes, or hair loss may occur in areas of severe skin damage. Recognition and management of these complications is important for optimal outcomes in cats with drug eruption.
