Distichiasis and ectopic cilia are conditions involving abnormally positioned hairs that emerge from or near the eyelid margin and contact the corneal surface, causing irritation and potential damage. While these conditions are relatively common in dogs, they are considerably rarer in cats, though they do occur and can cause significant ocular discomfort when present. Distichiasis refers to hairs arising from the Meibomian gland openings along the eyelid margin, whereas ectopic cilia are hairs that emerge through the conjunctival surface of the eyelid, typically on the inner (palpebral) surface facing the eye. Both conditions result in abnormal hairs contacting the sensitive cornea with each blink, creating ongoing irritation.
The causes of distichiasis and ectopic cilia are primarily developmental and often have a hereditary component. These conditions arise from abnormal differentiation of hair follicles in or near the Meibomian glands and eyelid tissue during fetal development or early life. Rather than forming normal Meibomian glands that produce the oily component of the tear film, some cells differentiate into hair follicles that produce fine hairs in abnormal locations. The exact genetic mechanisms are not fully characterized in cats, but the hereditary nature is suggested by breed predispositions and familial occurrence. Unlike acquired trichiasis, where normal hairs are misdirected due to eyelid scarring or malposition, distichiasis and ectopic cilia represent primary abnormalities in hair follicle location.
The impact of these conditions on feline ocular health varies from minimal to severe depending on the number, stiffness, and exact position of the abnormal hairs. Soft, fine distichia may cause little irritation and may not require treatment, while stiff hairs or those positioned to directly contact the central cornea cause significant discomfort and corneal damage. Ectopic cilia, because they typically exit through the conjunctival surface and directly abrade the cornea, tend to cause more severe problems than distichia. Clinical signs include squinting (blepharospasm), excessive tearing, ocular discharge, and corneal changes ranging from superficial irritation to ulceration. Without appropriate management, chronic corneal trauma can lead to scarring, vascularization, and visual impairment.
Distichiasis and ectopic cilia are treatable conditions, though management often requires permanent removal or destruction of the abnormal hair follicles to prevent regrowth and recurrence. Simple manual epilation (plucking) provides only temporary relief as hairs regrow. Definitive treatment involves various surgical and ablative techniques to destroy the offending follicles permanently. Prognosis for resolution of clinical signs is generally good with appropriate treatment, though some cases may require multiple procedures. Early recognition and treatment prevent cumulative corneal damage. Cat owners should be aware that signs of ocular irritation warrant veterinary evaluation, as identifying the cause enables appropriate management and prevents progressive corneal injury.
