Commonly co-occurring conditions with bronchitis share underlying inflammatory mechanisms or predisposing factors. Feline asthma is so closely related to chronic bronchitis that some veterinarians consider them a spectrum of the same disease, differing primarily in degree of bronchospasm. Upper respiratory infections may precede or coexist with bronchitis. Allergic rhinitis may occur alongside allergic bronchitis in cats sensitized to environmental allergens. Obesity frequently coexists with chronic bronchitis and may worsen respiratory symptoms. Heart disease occasionally occurs alongside respiratory disease and must be considered in differential diagnosis. Gastrointestinal disease, including inflammatory bowel disease, may share underlying inflammatory tendencies with bronchitis in some cats.

Conditions with similar symptoms that must be differentiated from bronchitis include other causes of coughing and respiratory difficulty. Pneumonia causes lower respiratory signs but typically with fever and more systemic illness. Heart disease can cause coughing and respiratory difficulty, particularly in later stages. Pleural effusion causes breathing difficulty but with different physical examination and radiographic findings. Lungworm and other respiratory parasites cause coughing and respiratory signs. Neoplasia affecting the airways or lungs causes progressive respiratory symptoms. Hairballs and esophageal disease can cause coughing-like behavior. Upper airway conditions including laryngeal disease may produce sounds mistaken for lower airway wheezing. Accurate diagnosis enables appropriate treatment selection.

Potential complications of bronchitis include progression and secondary problems. Airway remodeling from chronic inflammation can lead to permanent structural changes, reducing respiratory reserve even when inflammation is controlled. Bronchiectasis (abnormal bronchial dilation) may develop from chronic disease. Secondary bacterial infection can complicate inflammatory bronchitis, requiring antibiotic therapy. Severe acute exacerbations can progress to respiratory failure if not promptly treated. Corticosteroid side effects from long-term oral therapy may include diabetes and immunosuppression, though inhaled therapy reduces these risks. Pneumothorax (air leakage from lungs) occasionally complicates severe asthma or bronchitis due to air trapping and alveolar rupture. Recognition and prevention of complications improves long-term outcomes.
