In cats, this condition represents a medical emergency requiring immediate veterinary attention and aggressive treatment. The lungs are designed to receive only air, and when other materials enter the airways and lung tissue, they cause direct chemical injury, inflammation, and create an ideal environment for bacterial infection to develop. Aspiration occurs when the normal protective mechanisms that prevent material from entering the airways fail or are overwhelmed. These protective mechanisms include the epiglottis, which closes over the airway during swallowing, the cough reflex, and coordinated swallowing function. In cats, aspiration may occur due to various underlying conditions that impair swallowing, such as megaesophagus or neuromuscular disorders, during vomiting episodes, while under anesthesia or sedation, or during forced oral medication or feeding. The prognosis for aspiration pneumonia depends heavily on how quickly treatment is initiated, the amount and nature of aspirated material, and the underlying cause. With prompt, aggressive veterinary treatment including antibiotics, oxygen therapy, and supportive care, many cats can recover from aspiration pneumonia. However, the condition carries significant mortality risk, particularly when treatment is delayed, large amounts of material were aspirated, or underlying conditions prevent correction of the aspiration risk. Prevention focuses on identifying and managing predisposing conditions and taking appropriate precautions during anesthesia, medication administration, and feeding of at-risk cats.
