Renal amyloidosis is a serious progressive disease in cats characterized by the abnormal deposition of amyloid protein in the kidneys, leading to organ dysfunction and eventually kidney failure. Amyloid is an insoluble fibrous protein that accumulates in tissues when certain precursor proteins misfold and aggregate in ways the body cannot clear. In cats, this protein most commonly deposits in the kidneys, though the liver and other organs can also be affected. While amyloidosis can occur in any cat, it is particularly prevalent in certain breeds, most notably Abyssinians, Siamese, and Oriental Shorthairs, where it occurs as a familial condition with genetic inheritance patterns.
The development of amyloidosis in cats typically results from either genetic predisposition or chronic inflammatory conditions that trigger excessive production of amyloid precursor proteins. In hereditary forms, particularly the familial amyloidosis seen in Abyssinians, the condition develops due to inherited genetic mutations that cause overproduction or abnormal processing of amyloid proteins. In acquired forms, chronic infections, inflammatory diseases, or neoplastic conditions stimulate persistent production of serum amyloid A protein, which eventually converts to amyloid and deposits in tissues. Once amyloid accumulates in the kidneys, it progressively replaces normal tissue and impairs kidney function.
Renal amyloidosis has devastating effects on affected cats, causing progressive and irreversible kidney damage that ultimately leads to kidney failure. The amyloid deposits interfere with the kidneys' normal structure and function, preventing proper filtration of blood and regulation of fluid and electrolyte balance. As the disease progresses, cats develop signs of chronic kidney disease including increased thirst and urination, weight loss, decreased appetite, and eventual uremia. The condition can also cause nephrotic syndrome, characterized by severe protein loss in urine, fluid accumulation, and blood clotting abnormalities. Unlike some causes of kidney disease, the damage from amyloidosis cannot be reversed.
Unfortunately, there is no cure for renal amyloidosis in cats, and treatment focuses on managing symptoms and slowing progression. The prognosis is generally guarded to poor, particularly in cats with advanced disease or familial forms. Early detection through screening of predisposed breeds may allow earlier intervention and extended survival times. Treatment strategies borrowed from human medicine, including medications that may reduce amyloid production or deposition, are being explored in cats with variable results. Despite the serious nature of this disease, supportive care can maintain quality of life for affected cats during their remaining time, making veterinary partnership essential for optimal management.
