Heart murmurs in birds are abnormal sounds heard during cardiac auscultation that result from turbulent blood flow within the heart or great vessels. Unlike the normal heart sounds produced by valve closure, murmurs create whooshing, swishing, or blowing sounds that occur between or during the typical heartbeat sounds. These abnormal sounds may indicate underlying cardiac disease, though some murmurs are benign and have no clinical significance. Detection of a heart murmur in a pet bird warrants thorough investigation to determine its cause and clinical importance, as the finding may range from an incidental discovery to evidence of serious heart disease.
Heart murmurs develop when blood flow becomes turbulent rather than smooth and laminar. In normal cardiac function, blood flows silently through the heart chambers and valves. Turbulence occurs when blood velocity increases abnormally, when blood flows through narrowed or irregular openings, or when blood flows in abnormal directions such as backward through leaking valves. The resulting vibrations create audible sounds that trained veterinarians can detect with a stethoscope. In birds, their rapid heart rates and small body size make cardiac auscultation challenging, and murmurs may be subtle or intermittent.
The clinical significance of a heart murmur depends entirely on its underlying cause and the presence or absence of cardiac dysfunction. Innocent or physiologic murmurs occur in some healthy birds without any structural heart abnormality, often related to high blood flow states from excitement, anemia, or fever. Pathologic murmurs result from actual cardiac disease such as valvular abnormalities, congenital defects, or acquired heart conditions. The murmur itself causes no harm; rather, it serves as an indicator of underlying cardiovascular status. Some birds live normal lives with murmurs, while others may have progressive heart disease requiring treatment.
When an avian veterinarian detects a heart murmur, further evaluation is necessary to determine its significance and guide any needed treatment. Diagnostic workup typically includes echocardiography to visualize heart structure and function, radiographs to assess heart size, and possibly electrocardiography to evaluate heart rhythm. Based on findings, the veterinarian can characterize the murmur, identify any underlying disease, and recommend appropriate monitoring or treatment. Early detection and characterization of cardiac murmurs allows for timely intervention when needed and provides peace of mind when murmurs prove to be benign.
