However, and this is the important part, true blood in the urine does occur in rabbits and is always a sign that something needs veterinary attention. The difficulty is that red pigmented urine and bloody urine can look almost identical to the naked eye in the litter box. This is why understanding the context, knowing what your rabbit has been eating, and having a simple way to distinguish dietary color from actual blood matters so much. A vet visit is not always necessary, but knowing when it is can prevent a minor issue from becoming a serious one.
Rabbits have a unique urinary system compared to other common pets. They excrete excess calcium through their urine rather than through the gut like most mammals, which is why healthy rabbit urine is often cloudy or chalky white when it dries. This calcium-heavy urine means that rabbits are also prone to urinary conditions like bladder sludge and kidney stones that can involve blood in the urine. Seasonal changes in forage can cause red urine episodes that seem to come out of nowhere. Rabbits who graze on outdoor grass and weeds may encounter different plants at different times of year, and a new patch of dandelions or clover can trigger a color change. Pellet brands sometimes change their formulations slightly, and switching from one brand to another can cause temporary urine color shifts as well. Weather stress, changes in water intake, and even hormonal fluctuations in unspayed females have been anecdotally associated with red urine episodes.
True blood in the urine - called hematuria - has a completely different set of causes and always requires veterinary investigation. Urinary tract infections are relatively common in rabbits and can cause blood to appear in the urine along with increased frequency of urination, straining, and sometimes vocalization while urinating. Bladder sludge, which is an accumulation of calcium sediment in the bladder, can irritate the bladder wall and cause bleeding. Bladder stones can do the same and may also cause partial or complete urinary obstruction.
Uterine problems are a major cause of blood in the urine in unspayed female rabbits, and this is an important distinction because the blood may actually be coming from the uterus rather than the urinary tract. Uterine adenocarcinoma is extremely common in unspayed female rabbits over the age of three, with some studies suggesting that up to 80 percent of unspayed does will develop uterine cancer by age five. Uterine aneurysm, endometrial hyperplasia, and pyometra can all produce bloody discharge that mixes with urine in the litter box and appears identical to urinary bleeding.
Kidney disease, although less common, can also cause blood in the urine. Kidney stones, renal cysts, and kidney infections may all produce hematuria. Trauma to the kidneys or bladder from a fall or impact can cause temporary blood in the urine. Certain medications, particularly long-term NSAID use, may occasionally contribute to urinary tract irritation. Male rabbits can develop testicular or reproductive tract issues that produce bloody discharge as well. The range of possible causes is wide enough that a vet visit is the only reliable way to determine the source when true blood is suspected.
Section 3 Signs And Symptoms
Distinguishing dietary red urine from true hematuria starts with looking at the overall picture rather than just the color. A rabbit with plant pigment urine is otherwise completely normal - eating well, active, producing normal droppings, and showing no signs of discomfort. The red color appears uniformly throughout the urine rather than in streaks or spots, and it often has the same cloudy calcium-rich appearance as normal rabbit urine, just tinted red or orange. Within 24 to 48 hours of removing the suspected food from the diet, the urine typically returns to its normal color.
True blood in the urine may present differently. Blood sometimes appears as distinct red spots or streaks rather than a uniform color change. It may be concentrated at the beginning or end of urination rather than mixed evenly throughout. Fresh blood in urine often looks brighter red than the rust or brownish-red of plant pigment urine, though this is not a reliable distinction on its own, especially once the urine has been sitting in the litter box for a while. Blood clots in the urine - small dark red or brownish lumps - are a definitive sign that you are dealing with actual bleeding and not dietary pigment.
Behavioral signs that accompany true hematuria point toward a medical issue rather than a dietary one. A rabbit who is straining to urinate, making frequent trips to the litter box with little output, vocalizing or grunting while urinating, or sitting in a hunched posture in the litter box for extended periods is likely experiencing urinary tract pain. Wetness around the hindquarters from dribbling urine, increased licking of the genital area, and reluctance to move can all indicate urinary discomfort.
For unspayed female rabbits, bloody discharge from the vulva is a specific symptom to watch for. This blood may appear on the bedding, around the genital area, or mixed with urine in the litter box. Unspayed females over the age of two who produce any blood deserve a vet visit regardless of whether the blood is in the urine or from the reproductive tract, because the probability of uterine disease is high enough that investigation is always warranted.
A simple home test can help clarify the situation if you are unsure. Hydrogen peroxide applied to a suspected blood spot on a paper towel will fizz or bubble if actual blood is present, because the peroxide reacts with the hemoglobin enzyme. Plant pigments will not produce this reaction. This is not a perfect test - very dilute blood may not react noticeably - but a positive fizz gives you useful information to share with your vet. You can also collect a urine sample by placing a clean, non-absorbent surface in the litter box, like a clean plastic bag or a plate, and bringing the sample to your vet for analysis.

When you bring your rabbit to the vet for possible blood in the urine, the first diagnostic step is a urinalysis. X-rays are extremely useful for evaluating rabbit urinary issues because the calcium content of rabbit bladder sludge and stones makes them show up very clearly on radiographs. A normal rabbit bladder appears as a soft tissue shadow, while a bladder full of sludge appears bright white. Stones are visible as distinct dense objects. X-rays also allow evaluation of kidney size and shape. Ultrasound provides additional detail, particularly for assessing uterine conditions and soft tissue abnormalities that X-rays may miss.
Treatment depends entirely on the diagnosis. Urinary tract infections are treated with appropriate antibiotics based on culture and sensitivity testing when possible. Bladder sludge may be managed with increased hydration, diet modifications to reduce calcium intake, and in severe cases, bladder flushing under sedation. Bladder stones typically require surgical removal because rabbits do not respond well to the dietary dissolution approaches that work in some other species.
Uterine disease in unspayed females is treated with spay surgery, which in the case of cancer also serves as the primary treatment by removing the affected organ. The prognosis depends on whether the cancer has spread to other organs, which X-rays and ultrasound help evaluate before surgery. This is one of the strongest arguments for spaying female rabbits early - ideally between four and six months of age - because it virtually eliminates the risk of uterine cancer that becomes so common in older intact does.
For cases where the urine color turns out to be dietary, no treatment is needed at all. Your vet may suggest removing the suspected food for a few days to confirm the color returns to normal, then reintroducing it to see if the color change recurs. This simple elimination approach settles the question definitively in most cases and costs you nothing more than a few days of dietary adjustment.
Section 5 Home Care Prevention
Managing dietary red urine at home is straightforward once you know what causes it. Keep a mental note of what your rabbit eats before red urine episodes, and you will usually identify the trigger food within one or two occurrences. You do not need to permanently remove foods that cause red urine because the color change is cosmetically alarming but medically meaningless. However, knowing which foods cause it in your particular rabbit saves you future anxiety when the litter box turns colors again.
Encouraging good hydration supports overall urinary health and helps prevent the conditions that cause true blood in the urine. Always provide fresh, clean water - many rabbits prefer a bowl over a bottle because they can drink more naturally and in greater volume. Adding a second water station in a different location can increase water intake. Some rabbits drink more when their greens are served wet after washing rather than dried off. During warm weather, placing a couple of ice cubes in the water bowl may attract curious drinkers.
Diet plays a central role in preventing calcium-related urinary problems. The biggest dietary lever is pellet quantity - pellets are relatively high in calcium, and feeding excess pellets is a leading contributor to bladder sludge. Adult rabbits need only about an eighth to a quarter cup of pellets per day depending on size, with the vast majority of their diet coming from unlimited hay. Choosing a timothy hay-based pellet rather than an alfalfa-based one also helps control calcium intake for adult rabbits.
Spaying female rabbits is the single most effective preventive measure against uterine-origin blood in the urine. Given that the majority of unspayed females will develop uterine disease, spaying between four and six months of age effectively eliminates this entire category of risk. The surgery also prevents unwanted pregnancies, reduces hormone-driven behavioral issues, and may reduce the risk of mammary tumors. If you have an unspayed female and she produces any blood, consider the spay conversation with your vet sooner rather than later.
Regular litter box monitoring is your early warning system. Get into the habit of glancing at the litter box when you clean it, noting the color, consistency, and volume of urine and the quantity and quality of droppings. Changes from your rabbit's baseline deserve attention. A rabbit who suddenly produces noticeably less urine may be dehydrated or developing a blockage. A rabbit whose normally clear urine becomes persistently cloudy with thick white sediment may be accumulating bladder sludge. These observations become second nature and cost you nothing but a moment of attention during routine cleaning.

See your vet promptly if you observe blood clots in the urine, blood spots or streaks that do not look like uniform color change, straining or vocalizing during urination, frequent trips to the litter box with small amounts of output, or any bloody discharge from the genital area of an unspayed female. These signs point toward medical conditions that need diagnosis and treatment rather than dietary pigment.
A vet visit is also warranted if red urine persists for more than 48 hours after removing all suspected dietary triggers, if your rabbit is showing any other signs of illness alongside the color change, or if your rabbit has a history of urinary problems. Recurrent urinary tract infections, a previous diagnosis of bladder sludge, or a family history of kidney issues all lower the threshold for seeking veterinary evaluation of red urine.
Locating a rabbit-savvy vet before you need one makes these situations much less stressful. Not all small animal vets have extensive experience with rabbit urinary issues, and the diagnostic and treatment approaches differ enough from dogs and cats that rabbit-specific knowledge matters. Ask potential vets about their experience with rabbit urinalysis, bladder sludge management, and spay surgery. A vet who sees rabbits regularly will be familiar with the red urine question and can efficiently distinguish dietary causes from medical ones.
When the urine is uniformly red or orange, your rabbit is eating and drinking normally, producing normal droppings, and acting completely like themselves, you can reasonably monitor the situation at home for a day or two while removing suspected dietary triggers. If the color resolves, you have your answer and no vet visit is needed. The key is being honest with yourself about whether the situation is truly resolving or whether you are hoping it will resolve because a vet visit is inconvenient. When in doubt, a quick urinalysis is inexpensive, conclusive, and buys you peace of mind that is worth far more than the cost of the test.
