Neomycin is an aminoglycoside antibiotic that finds extensive use in small mammal medicine exclusively as a topical agent for treating superficial bacterial infections of the skin, ears, and eyes. Unlike other aminoglycosides such as gentamicin and amikacin that are commonly administered systemically, neomycin is considered too nephrotoxic and ototoxic for parenteral use in most species. The medication works through the same mechanism as other aminoglycosides, binding irreversibly to the bacterial 30S ribosomal subunit to disrupt protein synthesis and cause bacterial cell death. When applied topically, neomycin provides effective bactericidal activity against susceptible organisms while minimizing systemic absorption and the associated toxicity risks.
Discovered in 1949 and derived from Streptomyces fradiae, neomycin was one of the earlier aminoglycoside antibiotics developed. While its systemic toxicity limited its use as an injectable medication, neomycin's stability and effectiveness led to its widespread incorporation into topical preparations. Today, neomycin is found in numerous over-the-counter and prescription topical products, often combined with other antibiotics such as polymyxin B and bacitracin in triple antibiotic ointments, or with corticosteroids and antifungals in combination preparations for treating complex dermatological and otic conditions.
Neomycin is available in a variety of formulations designed for different applications in small mammal medicine. Topical ointments and creams are used for superficial skin infections, wounds, and abrasions. Otic preparations containing neomycin are prescribed for bacterial ear infections, often combined with antifungals and corticosteroids to address the multiple components of otitis. Ophthalmic preparations are available for bacterial conjunctivitis and other eye infections. Topical powders containing neomycin may be used for wound care in certain situations. The selection of formulation depends on the location and nature of the infection being treated.
The restriction of neomycin to topical use only in small mammals is critically important for owners and veterinarians to understand. Oral neomycin preparations exist for use in some large animal species, but oral administration in small mammals carries significant risks. Beyond the nephrotoxicity and ototoxicity concerns that affect all aminoglycosides, oral neomycin in some small mammal species could potentially contribute to disruption of normal intestinal flora. Topical application provides the benefits of this effective antibiotic while avoiding these serious systemic risks, making neomycin a valuable and relatively safe option when used appropriately.
