Ponazuril is a triazine antiprotozoal medication that has emerged as one of the most effective treatments for coccidiosis and several other protozoal infections in small mammals. This drug is the active metabolite of toltrazuril and works by inhibiting the mitochondrial and plastid functions of apicomplexan parasites, disrupting essential metabolic processes required for parasite survival and reproduction. Unlike older coccidiostats that primarily inhibit parasite development, ponazuril demonstrates coccidiocidal activity, meaning it actually kills coccidia organisms rather than merely preventing their reproduction, leading to faster parasite elimination and clinical improvement.

The medication gained popularity in exotic practice as veterinarians recognized its effectiveness against coccidia in species where traditional treatments had limitations or where alternative coccidiostats were unavailable or undesirable. Today, ponazuril represents a first-line treatment option for coccidiosis in many small mammal species.
Ponazuril is available primarily as an oral paste formulated for horses, which must be significantly diluted and carefully dosed for small mammal patients. Compounding pharmacies have become essential partners in exotic medicine for providing ponazuril in concentrations appropriate for small patients, with suspensions prepared at various strengths to accommodate species ranging from mice to rabbits. The medication's stability in compounded preparations and relatively long dosing intervals make it practical for outpatient treatment of exotic companion animals.
The safety profile of ponazuril in small mammals appears favorable based on extensive clinical use, though formal pharmacokinetic and toxicity studies specific to exotic species remain limited. Importantly for exotic practice, ponazuril does not disrupt intestinal flora in the manner of many antibiotics, making it safe for use in hindgut-fermenting species like guinea pigs, chinchillas, and rabbits where antibiotic-induced dysbiosis represents a serious concern.
