Gentamicin is considered safe for topical ophthalmic use in rabbits, as the route of administration results in minimal systemic absorption, avoiding the GI flora disruption concerns associated with many systemic antibiotics in this species.
The mechanism of action of gentamicin involves irreversibly binding to the 30S ribosomal subunit of bacterial ribosomes, causing misreading of messenger RNA and inhibiting protein synthesis. This action results in bactericidal activity, meaning the drug actively kills bacteria rather than simply inhibiting their growth. Gentamicin demonstrates excellent activity against most gram-negative bacteria including Pseudomonas aeruginosa, Escherichia coli, and Enterobacter species. It also has activity against some gram-positive organisms including Staphylococcus species, though this coverage is less comprehensive than its gram-negative spectrum.
Gentamicin ophthalmic is available in both solution (drops) and ointment formulations, each with specific advantages. The solution formulation provides rapid distribution across the ocular surface and is typically applied three to four times daily or more frequently for serious infections. The ointment formulation offers extended contact time with ocular tissues, making it useful for nighttime application or situations where prolonged drug presence is desired. Both formulations are generally well-tolerated by rabbits, though some may show transient mild discomfort upon application.

A rabbit-savvy veterinarian should diagnose the eye condition, determine whether bacterial infection is present, and assess whether gentamicin's spectrum is appropriate for the suspected pathogens.
