Cephalosporins are a large class of beta-lactam antibiotics that are FATAL to rabbits and must NEVER be administered under any circumstances. This class includes dozens of individual drugs spanning five generations, all of which share a mechanism of action that makes them lethal to hindgut fermenting species like rabbits. Common cephalosporins include cephalexin (Keflex), cefadroxil (Duricef), cefpodoxime (Simplicef, Vantin), cefovecin (Convenia), and ceftriaxone (Rocephin), among many others. Whether first-generation or fifth-generation, oral or injectable, all cephalosporins must be considered absolutely contraindicated in rabbits.
Cephalosporins are chemically and functionally related to penicillins, sharing the beta-lactam ring structure that gives both drug classes their antibacterial activity. Like penicillins, cephalosporins work by inhibiting bacterial cell wall synthesis, effectively killing susceptible bacteria. While this mechanism makes cephalosporins valuable for treating infections in humans, dogs, cats, and many other species, it creates the same fatal disruption of gut flora in rabbits that makes penicillins deadly. The broad antibacterial spectrum of cephalosporins destroys the beneficial bacteria that rabbits depend upon for digestion and survival.
The lethal effect of cephalosporins in rabbits results from the destruction of the delicate bacterial ecosystem in the cecum, the large fermentation chamber essential to rabbit digestion. Rabbits are obligate hindgut fermenters, meaning their entire nutritional strategy depends on cecal bacteria breaking down fiber and producing essential nutrients. When cephalosporins destroy these beneficial bacteria, pathogenic organismsâparticularly Clostridium difficile and Clostridium spiroformeârapidly proliferate and produce deadly enterotoxins. The resulting antibiotic-associated enterotoxemia progresses rapidly to death, typically within 24 to 72 hours of antibiotic administration.
Rabbit owners must understand that cephalosporin toxicity in rabbits cannot be prevented or mitigated. There is no safe dose, no safe route of administration, and no concurrent therapy that can protect against the fatal dysbiosis. The injectable cephalosporin cefovecin (Convenia), which provides long-acting antibiotic coverage with a single injection in dogs and cats, is particularly dangerous because its extended duration means toxic levels persist in the body for weeks. Any veterinarian who prescribes a cephalosporin for a rabbit is demonstrating inadequate species-specific knowledge, and owners must be prepared to refuse such prescriptions.
