Misoprostol, marketed under the brand name Cytotec, is a synthetic prostaglandin E1 analog used in horses primarily for gastroprotection and the prevention or treatment of gastrointestinal damage associated with non-steroidal anti-inflammatory drug (NSAID) use. Unlike acid-suppressing medications such as omeprazole or H2 blockers, misoprostol works through a fundamentally different mechanism by replacing protective prostaglandins that are depleted during NSAID therapy. This unique mechanism makes misoprostol particularly valuable in situations where NSAID-induced gastrointestinal damage is a concern, including both gastric ulceration and right dorsal colitis. The medication is not FDA-approved for equine use but is employed off-label under veterinary guidance.
The mechanism of action of misoprostol centers on its role as a prostaglandin analog. Prostaglandins, particularly prostaglandin E2 and prostacyclin, play crucial protective roles in the gastrointestinal tract by stimulating mucus and bicarbonate secretion, maintaining mucosal blood flow, and supporting epithelial cell renewal. NSAIDs work by inhibiting cyclooxygenase enzymes, which reduces inflammation but also depletes these protective prostaglandins, leaving the gastrointestinal mucosa vulnerable to acid-induced damage. Misoprostol replaces the depleted prostaglandins, restoring mucosal protection. Additionally, misoprostol has mild acid-suppressing effects, though this is not its primary therapeutic action.
Misoprostol is available in tablet form, which is the formulation used in horses. The tablets can be administered orally, either directly or crushed and mixed with a small amount of feed. The medication is typically given multiple times daily due to its relatively short duration of action. Unlike many gastroprotective medications, misoprostol is particularly useful for protecting both the glandular stomach and the hindgut, making it valuable in the treatment of right dorsal colitis, a serious condition affecting the large colon that is associated with NSAID toxicity.
The safety profile of misoprostol in horses requires careful consideration, as the medication has significant effects beyond gastroprotection. As a prostaglandin analog, misoprostol has smooth muscle stimulating properties that can cause gastrointestinal side effects including diarrhea and abdominal discomfort. More critically, misoprostol is absolutely contraindicated in pregnant mares due to its abortifacient properties—the medication can induce uterine contractions and pregnancy loss at any stage. This serious reproductive risk means that misoprostol must be handled with extreme caution in breeding operations, and women who are pregnant or may become pregnant should avoid contact with the medication.
