Histamine is one of the primary stimulants of gastric acid secretion, binding to H2 receptors and triggering acid production. In horses, oral administration is most common for outpatient treatment, while injectable forms may be used in hospitalized patients or those unable to take oral medications. The medication requires relatively frequent dosing due to its pharmacokinetic profile in horses, typically three to four times daily for optimal effect. This dosing frequency can be challenging for horse owners and represents one of the practical limitations of cimetidine use compared to once-daily omeprazole.
The general safety profile of cimetidine in horses is considered acceptable, though the medication is less commonly used now than in previous decades. Cimetidine has documented drug interactions that must be considered when prescribing, as it inhibits certain liver enzymes and can affect the metabolism of other medications. Veterinary supervision is essential when using cimetidine, both to ensure appropriate diagnosis and treatment of the underlying condition and to manage potential drug interactions. Horse owners should understand that while cimetidine can reduce acid secretion, the efficacy for healing established gastric ulcers in horses is lower than that achieved with omeprazole, and treatment decisions should be made in consultation with their veterinarian.
