At therapeutic concentrations, erythromycin is primarily bacteriostatic, stopping bacterial growth and allowing the dog's immune system to eliminate the infection. The medication demonstrates good activity against many gram-positive organisms, some gram-negative bacteria, and atypical pathogens including Mycoplasma and Campylobacter species. Erythromycin is available in multiple formulations for veterinary use, including oral tablets, capsules, and liquid suspensions, as well as ophthalmic ointment for eye infections. The oral forms are most commonly prescribed for systemic infections and gastrointestinal motility disorders, while the ophthalmic preparation is used for bacterial conjunctivitis and other eye conditions in dogs. Oral erythromycin is typically administered multiple times daily due to its relatively short duration of action, which can present challenges for owner compliance. The medication should generally be given on an empty stomach for optimal absorption, though this can increase the likelihood of gastrointestinal side effects in some dogs.
While erythromycin can be highly effective when used appropriately, it is associated with a higher incidence of gastrointestinal side effects compared to newer macrolide antibiotics. These effects are directly related to its prokinetic properties, as the same mechanism that makes it useful for motility disorders can cause nausea, vomiting, and diarrhea when that effect is unwanted.
