Lactulose is a synthetic disaccharide composed of galactose and fructose that serves as both an osmotic laxative and an ammonia-reducing agent in veterinary medicine. This medication is not digested or absorbed in the upper gastrointestinal tract but instead passes to the large intestine where colonic bacteria break it down into organic acids, primarily lactic acid and acetic acid. In guinea pig medicine, lactulose may be prescribed for two distinct therapeutic purposes: management of constipation and treatment of hepatic encephalopathy associated with liver disease.
The mechanism of action of lactulose as a laxative involves osmotic water retention in the intestinal lumen. When lactulose reaches the colon, bacterial fermentation produces organic acids that lower the colonic pH. This acidification draws water into the intestines through osmosis, softening stool contents and stimulating peristalsis. For guinea pigs experiencing constipation or difficulty passing feces, this gentle osmotic effect can facilitate bowel movements without the harsh stimulant effects of some other laxatives.
For hepatic encephalopathy, lactulose works through a different but related mechanism. In liver disease, the liver's ability to detoxify ammonia produced by intestinal bacteria is compromised, leading to accumulation of ammonia in the blood. Elevated blood ammonia affects brain function, causing neurological symptoms. Lactulose reduces blood ammonia by converting colonic ammonia to ammonium ions through acidification, trapping the ammonia in the colon for excretion. Additionally, the cathartic effect increases elimination of ammonia-producing bacteria and their substrates from the intestinal tract.
Lactulose is available as an oral syrup or solution that can be administered directly or mixed with food. The medication has a sweet taste that some guinea pigs tolerate well, though others may require gradual introduction or administration techniques to ensure adequate dosing. Veterinary guidance is essential for lactulose use in guinea pigs, as dosing must be individualized based on the specific condition being treated and the patient's response. Both underdosing, which may fail to achieve therapeutic effect, and overdosing, which can cause excessive diarrhea and electrolyte disturbances, should be avoided through careful veterinary supervision.
