By blocking calcineurin, cyclosporine prevents the activation of T-cells and the subsequent release of inflammatory cytokines that damage the lacrimal gland tissue and suppress tear production. Additionally, cyclosporine has direct lacrimogenic (tear-stimulating) effects that are independent of its immunosuppressive action. Through these combined mechanisms, cyclosporine can restore functional tear production in eyes where immune-mediated inflammation has impaired lacrimal gland function. Cyclosporine ophthalmic is available in various formulations including ointments and solutions at concentrations typically ranging from 0.2% to 2%. The veterinary product Optimmune contains 0.2% cyclosporine in an ointment base, while compounded preparations at higher concentrations may be prescribed for certain cases. For guinea pigs, the veterinarian selects the appropriate concentration based on the condition being treated, the severity of disease, and the individual patient's response. Ointment formulations provide prolonged contact time with the ocular surface, while solution formulations may be easier to apply in small eyes and cause less visual blurring.
Because cyclosporine addresses the underlying immune-mediated component of dry eye rather than simply providing symptomatic relief, it represents a fundamentally different therapeutic approach than artificial tears. While artificial tears must be applied frequently and provide only temporary moisture supplementation, cyclosporine can restore the eye's ability to produce its own tears, potentially reducing or eliminating the need for supplemental lubrication over time. However, response to cyclosporine treatment takes time to develop, typically several weeks to months, and artificial tears are usually continued during the initial treatment period.
