This mechanism requires active viral replication to be effective, meaning idoxuridine works best during acute infection when the virus is actively producing new copies of itself. The medication has no effect on latent virus residing dormant in nerve ganglia, which is why herpesvirus infections can recur even after successful treatment of active disease.
Idoxuridine ophthalmic is available as a compounded preparation in both solution and ointment formulations, typically at 0.1% to 0.5% concentration. The solution formulation requires frequent application, typically every four to six hours or even more frequently for severe infections, due to the medication's relatively short duration of action at the ocular surface. The ointment formulation provides longer contact time and may be preferred for nighttime use or when fewer applications are feasible. Compounding pharmacies prepare these formulations to veterinary specifications, and quality may vary between sources.
When used appropriately under veterinary supervision, idoxuridine demonstrates good efficacy against FHV-1 with an acceptable safety profile. The medication is most effective for superficial viral infections involving the corneal epithelium and conjunctiva. Deeper stromal disease may require alternative or additional treatments. Local irritation is the most common limiting factor for idoxuridine use, with some cats experiencing significant stinging or discomfort upon application.
