The most common indicators of terminal decline include a complete and sustained cessation of feeding that extends well beyond normal seasonal fasting, progressive and irreversible weight loss that continues regardless of ambient temperature and prey availability, chronic respiratory compromise that does not respond to antibiotic therapy or environmental correction, the growth of external or internal masses that impair normal function, and a marked reduction in responsiveness to environmental stimuli that previously elicited reliable behavioral reactions. A single one of these signs in isolation does not necessarily indicate a terminal state, but the convergence of two or more in a senior animal with a documented declining trajectory is a strong indicator that the end-of-life phase has begun.
Neurological deterioration, when present, is often the most distressing sign for keepers to observe. A Red-Tail Boa with progressive neurological disease may exhibit stargazing — a persistent upward orientation of the head — loss of righting reflex, corkscrew body posture, uncoordinated movement, and an inability to accurately strike at or constrict prey. The presence of advancing neurological compromise that prevents the animal from performing basic survival functions — feeding, thermoregulating, and navigating its enclosure — is a significant quality of life consideration.

Some Red-Tail Boas in terminal decline may persist for weeks to months in a stable but diminished state, while others deteriorate rapidly over a matter of days.
