Panophthalmitis is a severe, sight-threatening inflammatory condition involving infection of all structures within the eye, including the anterior chamber, vitreous cavity, and all ocular coats including the sclera. This condition represents one of the most serious eye emergencies in small mammals, characterized by aggressive bacterial, fungal, or occasionally parasitic infection that rapidly destroys internal eye structures. Unlike more localized eye infections, panophthalmitis involves the entire globe and typically results in permanent loss of vision in the affected eye, with treatment focused on preventing spread to other body systems and relieving pain.
Panophthalmitis can affect any small mammal species, occurring when bacteria or other pathogens gain access to the interior of the eye. All commonly kept small mammals including hamsters, rats, mice, gerbils, guinea pigs, chinchillas, ferrets, rabbits, hedgehogs, and sugar gliders are potentially susceptible. The condition may develop following penetrating eye injuries that introduce bacteria directly into the globe, spread from adjacent infected structures such as dental abscesses, or occur through bloodstream spread from infections elsewhere in the body. The relatively small size and high metabolic rate of many small mammals means that infection can progress extremely rapidly.
The impact of panophthalmitis on small mammal health is severe and often extends beyond the affected eye. The intense inflammation and infection cause significant pain that affects behavior, appetite, and overall wellbeing. Toxins from the infection can cause systemic illness, and bacteria may spread through the bloodstream to affect other organs. Even with aggressive treatment, the affected eye is almost always lost to the disease process. The primary treatment goals shift to preserving life, preventing spread of infection, and eliminating the source of ongoing pain and toxin production.
Treatability of panophthalmitis requires urgent intervention by a veterinarian, ideally one experienced in exotic small mammal medicine. While the affected eye cannot typically be saved, prompt treatment can prevent life-threatening complications. Aggressive systemic antibiotic therapy combats the infection, while surgical removal of the infected eye (enucleation) is usually necessary to eliminate the source of ongoing infection and pain. Early recognition and treatment dramatically improve survival outcomes, though the eye itself is rarely salvageable once panophthalmitis has developed.
