Liver abscess is a localized collection of pus within the liver parenchyma, resulting from bacterial infection that the body walls off in an attempt to contain the pathogens. This serious condition can develop in small mammals when bacteria reach the liver through the bloodstream, through direct spread from adjacent infected structures, or through ascending infection from the biliary system. Liver abscesses represent significant health threats in small mammals due to the liver's essential metabolic functions, the potential for abscess rupture causing peritonitis and septicemia, and the challenges of diagnosis and treatment in these small patients. Early recognition and aggressive treatment are essential for survival.
Liver abscesses can occur in any small mammal species, though they are relatively uncommon compared to other hepatic pathologies. Hamsters may develop liver abscesses secondary to dental disease, respiratory infections, or gastrointestinal infections that seed bacteria to the liver. Ferrets can develop hepatic abscesses as complications of various infections or following foreign body ingestion with intestinal perforation. Rats and mice may develop liver abscesses associated with respiratory disease or other bacterial infections common in these species. Chinchillas, hedgehogs, sugar gliders, and gerbils are all susceptible when conditions allow bacterial seeding of the liver.
The impact of liver abscess on small mammal health ranges from subclinical in very small, well-contained abscesses to severe systemic illness in cases with large abscesses or concurrent bacteremia. The infected focus within the liver compromises local hepatic function and can cause hepatomegaly that affects surrounding structures. Systemic effects from bacterial toxins and the inflammatory response cause generalized illness with fever, lethargy, and anorexia. The most serious complications occur if the abscess ruptures into the abdomen, causing peritonitis, or if bacteria continuously seed the bloodstream, causing septicemia. Small mammals' limited physiological reserves make them particularly vulnerable to these complications.
Treatability of liver abscess depends on the size and location of the abscess, the overall health of the patient, and how early treatment is initiated. Small, accessible abscesses may respond to prolonged antibiotic therapy alone or combined with surgical drainage. Large or multiple abscesses present greater treatment challenges and carry more guarded prognosis. Identifying and treating the source of infection is essential to prevent recurrence. Success requires accurate diagnosis, appropriate antibiotic selection based on culture when possible, and intensive supportive care from an exotic veterinarian experienced with small mammal medicine.
