Hydronephrosis refers to the swelling and dilation of the kidney resulting from obstruction to the normal flow of urine from the kidney to the bladder. When urine cannot drain properly through the ureter, it accumulates within the kidney's collecting system, causing progressive distension of the renal pelvis and eventually the entire kidney. This buildup of pressure damages the delicate kidney tissue responsible for filtering blood and producing urine, potentially leading to permanent loss of kidney function if not addressed promptly.
In small mammals, hydronephrosis may develop from various causes that block urine flow, including urinary stones lodged in the ureter, tumors compressing or invading the urinary tract, inflammatory strictures, or rarely congenital malformations. The condition may affect one kidney (unilateral) or both kidneys (bilateral), with bilateral involvement representing a more serious situation that can rapidly progress to life-threatening kidney failure. The small body size and delicate anatomy of exotic pets makes them particularly vulnerable to complete obstruction from even relatively small stones or masses.
Recognizing hydronephrosis in small mammals presents significant challenges because these prey animals instinctively hide signs of illness, and the symptoms when they do appear are often nonspecific. Animals may show gradual weight loss, decreased appetite, or reduced activity, but these signs could indicate numerous conditions. When unilateral hydronephrosis develops gradually, the opposite healthy kidney may compensate for function loss, allowing the condition to progress substantially before obvious illness develops. Bilateral obstruction or acute complete blockage produces more rapid deterioration.
Early detection and intervention significantly impact outcomes for small mammals with hydronephrosis, as kidney damage becomes progressively irreversible the longer obstruction persists. Diagnosis requires imaging studies to visualize the dilated kidney and identify the cause of obstruction. Treatment focuses on relieving the obstruction when possible, though the approach depends on the underlying cause, extent of kidney damage, and whether one or both kidneys are affected.
