Stomatitis with respiratory involvement represents a serious progression of oral infectious disease in which infection extends beyond the mouth to affect the upper respiratory tract, glottis, trachea, and potentially the lungs. This condition develops when infectious stomatitis (commonly known as mouth rot) is not recognized early or adequately treated, allowing pathogens to spread from the oral cavity into adjacent respiratory structures. The combination of oral and respiratory infection creates a complex disease process that is more difficult to treat and carries a more guarded prognosis than either condition alone.
All reptile species can develop stomatitis with respiratory involvement, though the condition is particularly common in snakes, bearded dragons, chameleons, and monitors. The anatomical relationship between the oral cavity and respiratory tract in reptiles facilitates spread of infection. The glottis (opening to the trachea) is located on the floor of the mouth, directly exposed to oral contents and any infection present in the mouth. Aspiration of infectious material during breathing or swallowing can introduce pathogens into the lower respiratory tract, potentially causing pneumonia.
The development of respiratory involvement from stomatitis significantly worsens the prognosis and complicates treatment. While uncomplicated stomatitis has a fair to good prognosis with appropriate treatment, extension to the respiratory system indicates more extensive infection, greater compromise of the animal's defenses, and increased treatment difficulty. The respiratory tract is more challenging to access for local treatment, and systemic illness is more likely once infection spreads beyond the oral cavity.
Successful management of stomatitis with respiratory involvement requires aggressive, multimodal treatment initiated promptly upon recognition of respiratory extension. Delays in treatment or inadequate therapy allow further disease progression and worsen outcomes. This condition represents a medical emergency requiring immediate veterinary attention from a reptile-experienced veterinarian. Understanding the serious nature of this condition motivates both early intervention for simple stomatitis before respiratory involvement occurs and aggressive treatment when extension has developed.
