Opisthotonus is a severe neurological sign characterized by extreme hyperextension of the head, neck, and spine, causing the reptile's body to arch dramatically backward. This distinctive posture results from sustained contraction of the extensor muscles and represents a serious indication of central nervous system dysfunction or severe metabolic disturbance. In reptiles, opisthotonus is considered a medical emergency that requires immediate veterinary intervention, as it typically indicates a life-threatening underlying condition that is affecting the brain, spinal cord, or neuromuscular system.
This dramatic postural abnormality can occur in any reptile species, though it is perhaps most commonly reported in lizards such as bearded dragons and leopard geckos, as well as in chelonians including aquatic turtles and tortoises. The presentation is unmistakable: the affected animal's head extends upward and backward, often appearing to gaze at the sky, while the spine curves dorsally and the limbs may extend rigidly. This posture may be sustained or may occur in episodes, sometimes accompanied by tremors, paddling movements of the limbs, or complete loss of normal motor function.
The impact of opisthotonus on the reptile's health is profound and multifaceted. Beyond the obvious postural abnormality, affected animals are typically unable to eat, drink, thermoregulate, or perform any normal behaviors. The severe muscle contractions can lead to exhaustion, hyperthermia, respiratory compromise, and physical injury. The underlying conditions that cause opisthotonus are themselves serious and progressive, meaning that without rapid intervention, the prognosis is often grave. The presence of this sign indicates that the pathological process has already significantly affected the central nervous system.
The treatability of opisthotonus depends entirely on identifying and addressing the underlying cause rapidly. When caused by reversible metabolic conditions such as severe hypocalcemia, prompt treatment with calcium supplementation may resolve the symptoms. Toxic exposures, if identified quickly, may be managed with decontamination and supportive care. However, opisthotonus caused by infectious encephalitis, severe head trauma, or advanced systemic disease carries a much more guarded prognosis. Regardless of the suspected cause, any reptile displaying opisthotonus requires immediate evaluation by a reptile-experienced veterinarian, as this is one of the most serious neurological emergencies in reptile medicine.
