Bacterial endocarditis is a serious infectious condition in reptiles characterized by bacterial colonization and inflammation of the endocardium, the inner lining of the heart, including the heart valves. This condition typically develops as a secondary complication of bacteremia, where bacteria circulating in the bloodstream from a primary infection site seed and establish infection within the heart. The bacteria adhere to the endocardial surface, particularly at sites of pre-existing damage or turbulent blood flow, forming vegetations composed of bacteria, inflammatory cells, fibrin, and cellular debris.
Bacterial endocarditis can occur in any reptile species but is most commonly diagnosed in larger lizards and chelonians where cardiac examination and imaging are more feasible. The condition appears to be relatively uncommon in clinical practice, though this may reflect underdiagnosis rather than true low prevalence, as the nonspecific clinical signs and difficulty of cardiac imaging in smaller reptiles likely result in many cases going unrecognized. When diagnosed, it is frequently at post-mortem examination, highlighting the challenges of recognizing this condition during life.
The impact of bacterial endocarditis on reptile health is profound and often ultimately fatal without aggressive intervention. The infection damages heart valve structure and function, leading to regurgitation or stenosis that compromises cardiac output. Pieces of infected vegetation can break off and embolize to other organs, causing secondary infections or infarction in the lungs, kidneys, liver, or other tissues. Chronic infection places enormous metabolic demands on the reptile while simultaneously compromising the cardiovascular system's ability to meet those demands. Immune complex deposition can cause additional organ damage distant from the primary cardiac infection.
Early detection and aggressive treatment are essential for any possibility of survival in reptiles with bacterial endocarditis. However, diagnosis is challenging and often occurs late in the disease course when treatment is less likely to be successful. Treatment requires prolonged courses of appropriate antibiotics, often administered parenterally, along with supportive care and correction of any underlying husbandry deficiencies. A reptile-experienced veterinarian with access to advanced diagnostics including echocardiography is essential for diagnosis and management. The prognosis remains guarded to poor even with aggressive treatment, making prevention through proper husbandry and prompt treatment of primary infections critically important.
