Sore hocks, medically known as pododermatitis or ulcerative pododermatitis, is an extremely common condition affecting the plantar surface (bottom) of rabbits' feet, typically the heel area of the hind feet though front feet can also be affected. This painful condition develops when prolonged pressure and friction cause damage to the skin and underlying tissues of the foot, resulting in hair loss, inflammation, ulceration, and potentially severe infection. Sore hocks represents one of the most preventable health problems in domestic rabbits, as it is primarily caused by inappropriate housing conditions and husbandry practices.
The condition occurs when the normal protective fur covering the rabbit's feet is worn away, exposing the skin to continued pressure and abrasion. Unlike dogs and cats, rabbits do not have protective foot pads, relying instead on thick fur to cushion and protect their feet. When this fur is damaged or worn away by rough or wire surfaces, wet or dirty bedding, or excessive pressure from obesity or lack of movement, the exposed skin becomes irritated, inflamed, and eventually breaks down. Once the skin is compromised, bacteria can enter and establish infection, transforming a manageable husbandry problem into a serious medical condition.
The impact of sore hocks on rabbit welfare is substantial. Even mild cases cause discomfort that can affect mobility, behavior, and quality of life. Rabbits may become reluctant to move, preferring to sit in one position to minimize foot pain. This decreased activity can lead to other health problems including obesity, muscle atrophy, and GI stasis from reduced movement and appetite. Advanced cases with deep ulceration and infection cause significant pain, potentially leading to septicemia if bacteria enter the bloodstream. Chronic sore hocks can progress to involve underlying bone, causing osteomyelitis that is extremely difficult to treat.
Treatment success depends heavily on addressing the underlying causes while providing appropriate wound care for existing lesions. Environmental modification to eliminate pressure and friction sources is essential and must be implemented regardless of the severity of existing lesions. Medical management includes wound care, pain relief, and antibiotics when infection is present. Most cases respond well to appropriate treatment, particularly when caught early, but severe cases may require prolonged intensive care. Prevention through proper housing and husbandry is far preferable to treating established disease, and awareness of risk factors allows owners to protect susceptible rabbits.
