Any rabbit showing signs of reduced grooming, mobility limitations, or abnormal stool should receive prompt attention to prevent perineal dermatitis development.

Related Conditions

Perineal dermatitis commonly co-occurs with and must be distinguished from several related conditions. Urine scald (hutch burn) causes similar skin damage primarily from urine exposure and often coexists with perineal dermatitis, as the same factors that cause fecal soiling frequently also cause urine contamination. GI stasis is a critical related condition because it both contributes to perineal dermatitis (through abnormal fecal production) and develops secondary to it (due to pain and decreased appetite). Any rabbit with perineal dermatitis must be monitored closely for signs of GI stasis, and treatment plans should include support for gut function. Pododermatitis (sore hocks) frequently accompanies perineal dermatitis because similar factors including obesity, reduced mobility, and unsanitary conditions affect both the perineum and the feet.

Several conditions may present similarly to perineal dermatitis and require differentiation. Rabbit syphilis (treponematosis), caused by Treponema cuniculi, produces crusty, scabby lesions around the genitals, nose, and lips that might initially resemble perineal dermatitis but require specific antibiotic treatment with penicillin. Myxomatosis, a serious viral disease present in some geographic regions, can cause genital swelling and discharge among other symptoms. External parasites including fur mites occasionally affect the perineal region. Allergic or contact dermatitis from bedding materials may cause perineal irritation. Treatable conditions like coccidia or other intestinal parasites may cause the diarrhea underlying perineal dermatitis and must be specifically identified and treated.

Potential complications of perineal dermatitis include serious and potentially life-threatening conditions. Fly strike (myiasis) is the most devastating complication, occurring when flies lay eggs in damaged, moist perineal tissue and resulting maggots feed on the rabbit's flesh. This can develop within hours in warm weather and is often fatal without immediate aggressive treatment. Secondary bacterial infection can spread from the skin to deeper tissues, causing abscesses or systemic sepsis. Chronic pain leads to GI stasis, which itself can be fatal within 24-48 hours if not treated. Hepatic lipidosis (fatty liver disease) develops if the rabbit stops eating. Stricture or scarring of the urethral or anal opening can occur with severe chronic cases. These potential complications emphasize the importance of prompt treatment and vigilant monitoring.
