Panophthalmitis is a severe, sight-threatening and potentially life-threatening condition in rabbits involving extensive inflammation and infection of all structures within and surrounding the eye. This condition represents the most severe form of intraocular infection, extending beyond the inner eye structures to involve the entire globe and surrounding orbital tissues. Unlike more limited infections such as conjunctivitis or keratitis, panophthalmitis affects every component of the eye including the cornea, anterior chamber, iris, lens, vitreous humor, retina, and sclera, as well as the tissues surrounding the eye. The result is a massively swollen, painful, non-functional eye that typically requires removal to prevent spread of infection and relieve the rabbit's suffering.
Panophthalmitis develops when infection or severe inflammation overwhelms the eye's natural defenses and spreads throughout all ocular structures. The condition may begin as a more limited infection, such as a corneal ulcer that becomes infected, or may develop following penetrating injury that introduces bacteria deep into the eye. In some cases, infection may reach the eye through the bloodstream from a distant site of infection, a process called hematogenous spread. Bacteria commonly involved include Pasteurella multocida, a ubiquitous organism in rabbit upper respiratory tracts, as well as Staphylococcus species, Streptococcus species, and various anaerobic bacteria. Once established, the infection spreads rapidly through the aqueous and vitreous fluids that fill the eye, reaching all internal structures.
The impact of panophthalmitis on the affected rabbit is severe and multidimensional. The eye becomes extremely painful due to inflammation, increased pressure, and tissue destruction. Vision in the affected eye is lost early in the disease process as the internal structures essential for sight are destroyed. The massive infection creates a source of bacteria and inflammatory mediators that can spread systemically, potentially leading to sepsis if not addressed. Secondary complications including gastrointestinal stasis commonly develop as the rabbit stops eating due to pain and illness. Without treatment, panophthalmitis is inevitably fatal either from uncontrolled infection, systemic spread, or secondary complications from not eating.
Treatment for panophthalmitis in rabbits almost invariably requires surgical removal of the affected eye, a procedure called enucleation. While this may seem drastic, attempting to save an eye with panophthalmitis is rarely successful and prolongs the rabbit's suffering. Enucleation removes the source of infection and pain, allowing the rabbit to recover. Rabbits adapt remarkably well to having one eye, maintaining normal activity and quality of life after healing. The prognosis following enucleation is generally good if the infection has not spread systemically, emphasizing the importance of prompt recognition and treatment. Early veterinary attention when eye problems first develop can prevent progression to this severe condition.
