Osteomyelitis is a serious bacterial infection of the bone that can affect rabbits of any age or breed, characterized by inflammation and destruction of bone tissue along with surrounding soft tissues. This condition represents one of the more challenging infections to treat in rabbits due to the difficulty of delivering adequate antibiotic concentrations to infected bone and the tendency for rabbit abscesses to form thick, caseous pus that walled-off areas cannot easily drain. Osteomyelitis most commonly affects the jaw bones secondary to dental disease, the bones of the feet and hocks secondary to pododermatitis, and any bone that sustains a penetrating wound or compound fracture. Without aggressive, prolonged treatment, osteomyelitis can lead to chronic pain, bone destruction, systemic infection, and potentially death.
The condition develops when bacteria gain access to bone tissue, most commonly through extension from adjacent soft tissue infection, direct contamination through wounds or surgical sites, or spread through the bloodstream from distant infection. In rabbits, Pasteurella multocida is the most frequently implicated bacterium, though Staphylococcus aureus, other staphylococcal species, and various anaerobic bacteria may also cause bone infections. Once bacteria establish in bone, they create areas of necrotic (dead) tissue called sequestra, which harbor bacteria protected from both the immune system and antibiotics. The infection stimulates new bone formation around the infected area, creating a shell of reactive bone called an involucrum. This combination of dead tissue, walled-off infection, and reactive bone changes makes osteomyelitis notoriously difficult to resolve.
The impact of osteomyelitis on affected rabbits is significant, causing chronic pain, loss of function in the affected area, and systemic illness as infection persists. Rabbits with jaw osteomyelitis may have difficulty eating, leading to weight loss and secondary GI stasis. Those with limb involvement become lame and reluctant to move. The chronic nature of the infection, combined with ongoing pain, significantly affects quality of life. If infection spreads systemically, septicemia can develop, which is life-threatening. Local spread can destroy increasing amounts of bone and involve adjacent structures. The psychological toll of chronic discomfort and illness affects the rabbit's overall wellbeing and behavior.
Treatment of osteomyelitis requires aggressive, multi-modal intervention typically including long-term antibiotic therapy, often surgical debridement to remove infected bone and tissue, and comprehensive supportive care. Treatment duration often extends for months, and even with optimal therapy, complete resolution is not always achievable. Early recognition and intervention improve outcomes, but the nature of rabbit abscesses and bone infections means that osteomyelitis is often advanced by the time it is diagnosed. A rabbit-savvy veterinarian with experience in treating complex infections is essential. The prognosis depends on the location and extent of infection, the causative organism, how early treatment begins, and the overall health of the rabbit.
