Osteomyelitis is a serious bacterial infection of bone that can affect any part of the rabbit's skeleton, though it most commonly involves the jaw bones secondary to dental disease, the long bones of the limbs following trauma or wound infection, and the feet as a complication of severe pododermatitis. This condition represents one of the most challenging infections to treat in rabbits due to the difficulty of delivering adequate antibiotic concentrations to bone tissue and the body's limited ability to clear infection from the relatively avascular bone structure. Without appropriate treatment, osteomyelitis causes progressive bone destruction, chronic pain, and potentially life-threatening systemic illness.
The condition develops when bacteria gain access to bone tissue, most commonly through direct extension from adjacent soft tissue infections, through open wounds or fractures, or via hematogenous spread through the bloodstream from infections elsewhere in the body. Dental disease is an extremely common source of jaw osteomyelitis in rabbits, as tooth root abscesses can extend into the surrounding bone. Soft tissue infections of the feet, including severe sore hocks and nail bed infections, can progress to involve underlying bones. Bite wounds, surgical sites, and traumatic injuries all provide potential entry points for bone infection.
The impact of osteomyelitis on rabbit health is significant and multifaceted. The infection causes bone destruction, leading to structural weakness and potential pathological fractures. Chronic pain from bone infection reduces quality of life, decreases mobility, and suppresses appetite. Reduced food intake in rabbits quickly leads to GI stasis, which can become life-threatening within 24 to 48 hours. The metabolic demands of fighting chronic infection deplete the rabbit's reserves. Bacteria in the bone can serve as a reservoir for recurrent systemic infections. In severe cases, toxins from the bacteria and the body's inflammatory response can cause sepsis and organ failure.
Treatment of osteomyelitis requires aggressive, prolonged intervention and carries guarded prognosis even with optimal care. Long-term antibiotic therapy lasting weeks to months is typically necessary, using antibiotics specifically chosen for bone penetration and confirmed effectiveness against the causative organism. Surgical debridement to remove infected bone may be required. Supportive care maintains the rabbit's nutrition and comfort throughout the extended treatment period. Despite these interventions, cure is not always achievable, and some cases require amputation of affected limbs or, when the jaw is severely involved, consideration of quality of life and humane euthanasia.
