GI stasis frequently develops secondary to the stress, pain, and decreased appetite caused by mange infestation. Any rabbit with mange should be monitored closely for signs of gut slowdown including reduced eating, smaller or absent fecal pellets, and lethargy. Supporting gut function through ensuring adequate food intake is an essential part of mange management. Secondary bacterial skin infections occur when the skin barrier is compromised by mite damage and scratching, requiring antibiotic treatment alongside antiparasitic medication. Ear mites can progress to cause otitis media and otitis interna, with potential permanent vestibular damage resulting in head tilt.

Several conditions may be confused with mange and must be properly differentiated. Dermatophytosis (ringworm) is a fungal infection that causes hair loss and scaling similar to some mange presentations; fungal culture differentiates the two conditions. Treponematosis (rabbit syphilis) produces crusty lesions around the face and genitals that might resemble sarcoptic mange lesions. Allergic dermatitis from bedding, hay, or environmental allergens can cause itching and skin changes. Bacterial skin infections without mite involvement may produce crusting and hair loss. Other causes of ear disease including bacterial otitis and foreign bodies cause ear discharge and head shaking. Proper diagnosis ensures appropriate treatment is provided.

Potential complications of mange extend beyond the direct mite damage. Chronic severe scratching leads to self-inflicted wounds that can become secondarily infected. Aural hematoma (blood accumulation within the ear pinna) can result from intense head shaking with ear mites. Extension of ear mite infection to middle and inner ear structures causes serious vestibular complications. Severe chronic mange leads to debilitation, weight loss, and general decline in health. Immunosuppressed rabbits may develop overwhelming infestations that become life-threatening. Zoonotic transmission to human caregivers causes temporary itchy rashes until the mites die without completing their life cycle. These potential complications emphasize the importance of prompt diagnosis and treatment.
