Buphthalmia, also known as buphthalmos or ox eye, is a condition in rabbits characterized by abnormal enlargement of the eyeball resulting from elevated intraocular pressure during the developmental period when the eye structures are still pliable. The term comes from Greek roots meaning ox eye, reflecting the dramatically enlarged appearance of affected eyes. In rabbits, buphthalmia most commonly occurs as a congenital condition associated with developmental abnormalities of the aqueous drainage pathways, though it can also develop secondary to acquired glaucoma in young rabbits whose eyes have not yet fully matured and hardened.
The primary cause of congenital buphthalmia in rabbits is goniodysgenesis, a developmental abnormality of the iridocorneal angle where aqueous humor normally drains from the eye. When the drainage structures fail to develop properly, aqueous humor accumulates within the eye, causing elevated pressure. In the young rabbit's eye, which has not yet developed the rigid structure of an adult eye, this elevated pressure causes the globe to stretch and enlarge. This condition has been particularly well documented in certain lines of white rabbits, especially New Zealand Whites, where it appears to be inherited as an autosomal recessive trait.
The impact of buphthalmia on affected rabbits is significant. The enlarged eye is typically blind or has severely compromised vision due to damage to the retina and optic nerve from the chronically elevated pressure and stretching. The cornea is often cloudy due to edema from the stretching, further impairing any remaining vision. Beyond vision loss, the enlarged, prominent eye is more vulnerable to trauma and exposure keratitis, where the cornea dries out because the lids cannot adequately protect it. The condition can cause discomfort, and affected rabbits may face challenges with depth perception and navigation.
Management of buphthalmia focuses on controlling intraocular pressure when possible, protecting the enlarged eye from complications, and maintaining comfort. Medical treatment with pressure-lowering medications may provide some benefit, though the response in congenital buphthalmia is often limited. If the eye is painful or develops complications, enucleation may be recommended to eliminate discomfort. With appropriate management, rabbits with buphthalmia can live comfortable lives, though visual impairment in the affected eye or eyes is typically permanent.
