Bladder sludge, also known as calcium sludge or hypercalciuria, is a common urinary condition in domestic rabbits characterized by accumulation of calcium carbonate sediment in the bladder. This thick, paste-like material ranges from gritty sand-like particles to dense sludge resembling toothpaste in consistency. Bladder sludge develops because of rabbits' unique calcium metabolism, where dietary calcium is absorbed from the intestines regardless of the body's needs and excreted primarily through the kidneys into the urine. When urinary calcium levels exceed the bladder's ability to flush the mineral out with normal urination, deposits accumulate in the bladder, causing irritation, infection risk, and potentially serious complications including bladder stones and urinary obstruction.
The development of bladder sludge relates directly to how rabbits process calcium differently from most other mammals. In most species, intestinal calcium absorption is regulated based on physiological needs. In rabbits, however, calcium absorption is largely passive and unregulated, meaning that dietary calcium intake directly correlates with urinary calcium excretion. Diets high in calcium, including excessive alfalfa hay, calcium-rich vegetables, and high-calcium pellets, result in increased urinary calcium levels. When combined with inadequate water intake leading to concentrated urine, insufficient exercise affecting bladder emptying, or conditions that impair complete voiding, calcium accumulates in the bladder faster than it can be eliminated.
The impact of bladder sludge on affected rabbits ranges from asymptomatic in mild cases to severely debilitating when sludge causes significant bladder irritation or urinary complications. Sludge acts as a chronic irritant to the bladder wall, causing inflammation and predisposing to bacterial cystitis. Thick sludge may obstruct the urethra, particularly in male rabbits with their narrower urinary passage, creating a life-threatening emergency. The discomfort associated with bladder sludge commonly causes decreased appetite, which in rabbits rapidly becomes dangerous due to their need for continuous gut motility. GI stasis can develop within 24 hours of significant appetite reduction, adding a secondary life-threatening complication. Additionally, straining to pass sludge causes urine dribbling and scalding of the perineal skin.
With appropriate management including dietary modification, increased hydration, and supportive care, many rabbits with bladder sludge can be maintained comfortably with good quality of life. However, this condition typically requires lifelong management rather than cure, as rabbits remain predisposed to sludge accumulation due to their calcium metabolism. A rabbit-savvy veterinarian experienced in exotic animal medicine is essential for proper diagnosis, treatment of complications, and development of an effective long-term management plan. Early detection and intervention improve outcomes, though rabbits hide illness as prey animals and may not show obvious symptoms until sludge accumulation is significant.
