Equine protozoal myeloencephalitis may affect cranial nerves controlling swallowing but usually presents with additional gait abnormalities. Physical causes of dysphagia including esophageal obstruction from choke, severe dental disease, temporomandibular joint dysfunction, and masses in the throat region must be excluded through careful examination.

Potential complications beyond those directly related to the inability to eat include injuries sustained due to weakness or incoordination as the condition progresses. Horses that become recumbent from weakness may develop pressure sores, myopathy from prolonged lying down, and respiratory compromise. The psychological distress of being unable to eat while remaining hungry and alert contributes to a poor quality of life that supports the recommendation for humane euthanasia. Secondary infections may develop in debilitated horses with compromised immune function. Owners should understand that these complications are inevitable consequences of the underlying irreversible brain damage and that preventing them requires the difficult decision to euthanize the horse before they develop.
