Purpura hemorrhagica is a serious immune-mediated vasculitis that affects horses, characterized by widespread inflammation of small blood vessels throughout the body. This condition occurs when the horse's immune system produces antibodies that attack the walls of blood vessels, leading to increased vascular permeability and the leakage of blood and fluid into surrounding tissues. The result is dramatic subcutaneous edema, hemorrhages, and potentially life-threatening complications if left untreated. Purpura hemorrhagica represents one of the most striking and concerning complications that can follow bacterial infections in horses, particularly those caused by Streptococcus equi subspecies equi, the bacterium responsible for strangles.
This condition can affect horses of any age, breed, or discipline, though it most commonly occurs in horses that have recently recovered from strangles or have been exposed to Streptococcus equi bacteria. Horses that have experienced multiple exposures to these bacteria or have received strangles vaccinations while harboring subclinical infections appear to be at increased risk. The condition has also been reported following other bacterial infections, including those caused by Streptococcus equi subspecies zooepidemicus and Corynebacterium pseudotuberculosis, as well as following viral respiratory infections in some cases.
The impact of purpura hemorrhagica on equine health can be devastating without prompt recognition and aggressive treatment. The massive fluid shifts that occur can lead to severe dehydration, electrolyte imbalances, and organ dysfunction. Extensive skin necrosis may develop in areas of severe edema, particularly on the limbs and ventral abdomen, leading to secondary infections and prolonged healing times. In severe cases, the inflammatory process can affect internal organs including the kidneys, gastrointestinal tract, and respiratory system, potentially resulting in fatal outcomes.
Despite its serious nature, purpura hemorrhagica is treatable when identified early and managed aggressively with appropriate immunosuppressive therapy, supportive care, and treatment of any underlying infections. The prognosis varies considerably depending on the severity of clinical signs at presentation, the speed of treatment initiation, and the horse's response to therapy. Early recognition of the characteristic clinical signs, including limb swelling and petechial hemorrhages, is essential for achieving the best possible outcome and preventing the potentially devastating complications associated with this immune-mediated condition.
